Your first visit
Keep this page for your appointment. Visits are at the Bowie clinic or at home. A family member or caregiver is welcome either way. Clinic and mobile, Prince George's County.
What to bring
- Photo ID
- Insurance card, front and back, if you have one
- Referral or prescription, if you already have it
- A list of your current medications
- Glasses, hearing aids, or any walking aid you use
- Comfortable clothes you can move in, and supportive shoes
What to expect on your first visit
Your first visit can feel unfamiliar, whether you are seen at the clinic or at home. Here is exactly what happens so you feel prepared and comfortable.
- Your clinician introduces themselves, reviews your medical history, and discusses your goals and concerns.
- A thorough assessment is done at the clinic or in your home to look at how you are managing and what you can do now.
- Together with your input, a personalized care plan is created based on your physician’s orders and your specific needs.
- Your clinician explains visit frequency, what to expect going forward, and answers your questions.
- We coordinate with your physician and any other providers so everyone is aligned on your plan of care.

Questions I want to ask
