What to have ready

- A list of medications and the doctors involved in your care
- The address for a home visit, if you want one
- Questions you want answered on the first call
What this form does not do
- It does not book the visit by itself
- It is not the place for a diagnosis or an insurance ID number
- It does not replace a call if the need is urgent
